We welcome referrals from professionals and loved ones

 

HARP Private Rehab accepts referrals for its residential recovery program from a wide range of sources,

We accept referrals from:

  • General Practitioners (GPs)
  • Psychologists and Psychiatrists
  • Allied Health Professionals (e.g. social workers, counsellors, physiotherapists)
  • Hospitals and Health Services
  • Legal representatives and case managers
  • Family members, carers, and loved ones
  • Community-based organisations
  • Employers and EAP providers

Each referral is thoroughly evaluated by the Intake & Clinical Team. If the patient is deemed suitable, HARP will contact them to discuss treatment options and next steps

  • The person being referred must be aware that a referral is being made on their behalf.
  • Please complete this form with as much accurate detail as possible to allow our team to make an informed assessment.

What happens next?

  • Referral submission – You complete and submit the form.
  • Clinical review – Our Intake & Clinical Team reviews the referral.
  • Assessment & outreach – If suitable, we’ll contact the individual to explore treatment options and arrange intake.
